We provide Person-centred care to ensure that you are at the centre of everything done with and for them. We will always consider your wishes, needs, life circumstances and health choices. Our Home care services are designed to promote your independence and wellbeing to ensure a healthy and happy life.

Our Home Care services are mainly aimed at the elderly as well as adults with a physical disability, a learning disability or a mental health problem.

How It Works

Our care assessment team will come to you to carry out a free no obligation assessment of your needs to determine the most suitable care package for you or your loved one.

We will then agree with you an outcomes based support and care plan based on the assessment of needs. The support plan will set out individual outcomes or goals for you or your loved one, in particular identifying how you can be enabled to become more independent.

If you are happy with the proposed plan we will arrange a start date for care to commence.

Our carers will support you with;

  • Personal Care         
  • Moving and Handling
  • Medication
  • Meal Preparation and Menu Planning
  • General support tasks such as Bed Making and Laundry
  • Communication, Social Care and Social Integration
  • Daily Health, Safety and Well-Being
  • Attending Appointments and Collecting Prescriptions

Our Home Care Service aims to;

  • Promote the independence of Service Users through an enabling approach;
  • Work with Service Users to achieve the outcomes in their support plans and maximise their independence
  • Support Service Users to re-engage/engage with their family/family, their interests and community services;
  • Support Service Users to improve their health and wellbeing.
