How It Works
In the first instance we carry out an assessment of needs to determine the individual support required and outcomes will be stated in a personal support plan and what you would have told us is important to you. All support plans are personalised and tailored to individuals.
Depending on your needs we will determine how often we come to support you and what areas you need support with.
When you needs change, we will review your support plan to ensure that the change is reflected and support is adjusted to suit appropriately.
